Exactly this question was examined in the doctoral thesis on the distalization of upper first molars with headgear depending on the eruption stage of the second molars, completed in 2007 at the University of Bonn. Experimental measurements and a finite element model recorded the movements of both molars. Pendulum tipping occurs because the spring force acts occlusal to the molar's center of resistance. When is which effort worthwhile? For an adolescent with a space deficiency of just a few millimeters, the dentally anchored option is often enough. Check the radiograph beforehand to see whether the tuberosity region leaves enough room for the molars.

Once the molars sit in Class I, the trickier half begins. Without a holding element, they tend to drift mesially again as soon as you retract premolars and incisors.
